Who we are

PackHistory (packhistory.com) is a free tool that helps people and their care teams work out a cigarette smoking history in pack-years. It is run by Precare. You can reach us at info@packhistory.com.

Your questionnaire answers

The answers you type, such as your date of birth and your smoking history, are used only inside your browser to calculate the result.

• They are not sent to our server or to anyone else.
• They are not saved in cookies or in your browser’s storage.
• They are erased when you close or reload the page, when someone taps “Start over”, or after 30 minutes without activity.

Because we never receive your answers, we hold no health information about you.

The summary, the note and the PDF

The summary for your care team, the note for your medical record and the PDF are all created on your device. If you or your care team copy the note, print the page or save the PDF, that copy stays with you or with your clinic. We never receive it. What your clinic does with it is covered by the clinic’s own privacy practices.

What our server records

Like most websites, the server that hosts PackHistory keeps standard technical logs: the IP address of the device that asked for a page, the date and time, the page requested, and the type of browser. We use these logs only to keep the site secure and working. They are never combined with anything you type in the questionnaire. The raw logs are deleted within two months. Monthly summaries built from them, which can include IP addresses, are kept longer.

Cookies and tracking

PackHistory has no advertising, no analytics trackers and no social media pixels. The site may set a cookie that is strictly needed to run it, such as one that remembers your language. That cookie contains nothing about you or your answers.

The video on the home page is hosted by Vimeo. It loads only when you press play, and it plays in Vimeo’s do-not-track mode, which does not set cookies or record your viewing session. Once it loads, Vimeo receives your IP address, as any website you visit does.

If you contact us

If you send us an email, we use your address and your message only to reply to you. Please do not include health information in an email.

Links to other websites

PackHistory links to other websites, such as Should I Screen and Smokefree.gov. Those sites have their own privacy policies, and we are not responsible for how they handle information.

Children

PackHistory is meant for adults. It is not directed to anyone under 18.

Not medical advice, not a medical record

PackHistory is a decision-support tool. It is not your health care provider and it does not keep medical records. Talk to your care team about your results.

Changes to this policy

If we change how the site works, we will update this page and the date at the top.
